If you have any questions about your first visit or our services in general, please see go to either Frequently Asked Questions (FAQ) or Prepare for you first visit.

If you do not currently have a prescription, go to download a referral form and this prescription will be valid if signed by your physician, nurse practitioner, physician's assistant, chiropracter, podiatrist, or dentist (i.e. in the case of a TMJ disorder).
